I know the playing length is 40.50, but I’d like the playing length to be 41.25. So I’m going to order a new shaft but how do I measure? With or without the adapter,

Assuming you already have the TSR2, if you're going to order a longer shaft most respectable suppliers will ask what the desired playing length is. They will set it up for you grip to tip, but there are other considerations you need to check. Be sure to order a few head weights so you can adjust your overall swing weight to your preference. Usually you will go lighter in the head weight when you lengthen the shaft so maybe a -2 and -4 gram. Keep in mind that you will also be changing the lie angle. If you do it yourself, buy the shaft, check for tip trimming specs and cut the tip first, install the tip, and cut the butt end to desired length. Unless you're using an experienced club fitter to do the work, or you are experienced yourself, you could really mess up the characteristics of the club. Best to have a qualified fitter help you determine if this is what you really want.

Playing length isn't a measurement of the shaft with or without the adaptor. Playing length is the measurement of the completed club and is measured without the grip from the end of the butt through to the floor/ground. Purchase a 48 inch metal ruler and lean it against a wall with the club laying against it while making sure the club is properly soled. The installed grip will add 1/8" to 1/4" depending upon the depth of the grip cap.
